I hereby request that Vero Classical Ballet allow my (son, daughter, ward hereby referred to as student) to register and fully participate in the Vero Classical Ballet classes/camps designated below. I understand that my student cannot participate unless tuition is paid, and that no refunds will be issued after the first class.

I hereby give Vero Classical Ballet permission to use my child’s image for promotional materials.
